Class I and II Medical Record Form (BSA 34414)  

Class 1 Annual, Class 2 every 3 years. Good for youth and adults under 40 for activities. Can't be used for Adventure Bases or Jamborees (use form 34412A).

Class III Medical Record Form (BSA 34412A)

All Class 3 activities require a health examination within the past 12 months by a licensed health-care practitioner.* This includes youth and adult members participating in high-adventure activities, athletic competition, and world jamborees. Annually, this form is to be used by adults over 40 for all activities requiring a physical examination and applies to allWood Badge participants/staff regardless of age.
